With each first visit, typically there are several forms to fill out which can be very time consuming.  For your convenience, and to expedite your first visit with us, we have provided the forms for you, here on the website, that you can print, fill out and bring with you to your first appointment.  You can either click on each individual form or you can click the "Print ALL First Visit Forms" to print all 15-pages at once.

We accept Medicare, Medicaid and most major insurances.
